Performance Appraisal System

I have to make Performance Appraisal system for my company(800 employees).
Employees will fill their Key Result Areas and on the basis of that the annual reports will be generated for helping HR department like Employee and particular department Summary, Ranking, comparison, history, etc.
Also previous year data need to be stored.
What tables and columns in database needed for storing past year data? Should I include date in main employee detail table which contain name, designation, manager name etc.
Please help...
