Mysupa Feedback is a multi-platform compatible script that will let you have a customer feedback, or comments form on your website. Submission forms are name, email, URL, Where did you here about us?, your feedback. The script can be easily customized to suit your own website design, and also has the feature to display a random user comment anywhere on your site using SSI. Has a page which displays all comments, with persons name and website. Very customizable script, can change form names, change comment layout and more. Also has an admin.
