BNB Survey Script
BNB Survey Script is a Unix compatible script that has cookies to prevent multiple votes, or to limit how often a person can vote. Can also capture e-mail address and comments. Survey can be taken from within a web page, and results can be included on a page using SSI. The script is fairly easy to install and there are no limits on the number of multiple choice questions per survey.
